If you were a full-time employee or were covered under PHDC’s health plan in 2025, you have a right to receive a copy of your 2025 Form 1095-C, which contains information about the health insurance you were offered.
Although the IRS no longer requires us to automatically mail this form to all employees/participants, you are entitled to a copy upon request. To request a copy of your 2025 Form 1095-C, please contact us by:
